Whipple’s Disease

  • – This is a multi-system disease
  • Organism : Trophyrema whippleii
  • – Small bowel is involved and GI symptoms are prominent
  • – Typically
  •               • Mucosa of the small intestine contains PAS(periodic acid schift) positive macrophages with disruption of villi
  • – Submucosa contains T.whippleii seen by electron microscope
  • – In AIDS, infection with MAI in intestine also contains PAS +ve foamy macrophage. So,it should be distinguished carefully
  • Extraintestinal symptoms: Fever, cough, arthritis, dementia, headache,muscle weakness

Treatment:

  • – Antibiotic for years
